A: There is no actual form to fill out. You should just request the copy in writing. On the request, please include: Name(s) on return, social security(s) on return, current mailing address, daytime phone #, your signature and copy of photo ID. You can either fax to Taxpayer Assistance at (502) 564-3392 or you may mail to Kentucky Department of Revenue, PO Box 181, Station 56, Frankfort, KY 40602.
Q: Is there a charge for the copy?
A: No there is no charge. Once the request is received, it can take up to a week to process.
